Child Guidance Clinic

Welcome to Child and Adolescent Psychiatry on the MAMC web site. We hope that the information that you find here will aid in the care and treatment of your child.

Information for Patients:

The Child Guidance Clinic is located on the first floor of the Nursing Tower between the Chapel and the Blood Donor Center. MAP

IMPORTANT INFORMATION FOR NEW PATIENTS:
Prior to appointments, intake paperwork should be completed.  If you would like to review and/or complete this prior to your visit, the following required forms are availble here:

-Standard Privacy Act 
-Behavioral Health Informed Consent to Treatment
-Behavioral Health Intake

Additional forms are available that your provider may request be completed.

Information for referring patients to the Child and Adolescent Psychiatry Service

Families must have a consult written from their Primary Care Manager for the child or adolescent of concern. Appointments for this can be made through TRICAREExternal Link, Opens in New Window at 1-800-404-4506.  That consult will be reviewed by our treatment team and the family will be contacted for a referral to the community or to be evaluated in our clinic in a timely manner.  Families who prefer to be seen in the community can self-refer through Tri-WEST 1-888-TRI-WEST (1800-874-9378) without a referral from MAMC.
