• Where is my Medical Record?
  
  Your original military medical record is maintained at your nearest military medical treatment facility regardless of whether or not you are enrolled in TRICARE. At Madigan, TRICARE enrolled PRIME patient’s records are kept in their Primary Care Clinic (Family Practice, Pediatrics, or Internal Medicine). All other records are in the medical records area across from the Pharmacy in the Medical Mall, first floor.  
  

• How Do I Get a Copy of my Medical Record?
  
  Should you require a copy of your Inpatient or outpatient record, the Medical Correspondence Section of Patient Administration Division will make the copies for you. You may request a copy of your medical record from the main medical records room near the pharmacy or from Correspondence, which is located in the admissions area, first floor of the Nursing Tower. Please be patient, copies cannot be produced immediately upon request. You will be told when you can expect the copies to be completed.
