Sick call:

Report to your unit battalion aid station for prescreening. Those needing to see a provider will be sent with a signed sick call slip to Okubo Clinic Monday – Friday, 6:30 a.m. – 7:30 a.m.

Immunizations:

Walk-in to the Immunizations Room at Okubo Clinic is available Monday – Friday, 8 a.m. – 2:30 p.m. The Immunizations room is closed on Thursdays from 7:30 a.m. – 11 a.m. for Sergeant’s Time Training.

HIV:

Walk-in to the Lab at the Okubo Clinic Monday-Friday, 7:30 a.m. - 3 p.m.

Vision:

Vision Screenings are available on a walk-in basis at Okubo Clinic or your Battalion Aid Station.

Hearing:

Annual Hearing Exams are done by appointment at the Madigan Annex. To book, please call the TRICARE Appointment Center at (800) 404-4506 and request a “well” exam. The Lewis-North Hearing Booth, Bldg. 13815 is also open Monday, Wednesday and Friday from 1 p.m. - 2:45 p.m. and by appointment. No hearing testing is performed DoD-wide on Tuesdays as calibration, upgrades and maintenance processes are performed.

PHAs:

PHAs are completed on a walk-in basis at your Battalion Aid Station. BASs conduct PHAs during regular duty hours. Your PHA will be completed by a unit medic or by your unit PA. Any acute issues will be triaged accordingly.

Individual Medical Equipment:

IME updates are completed by unit medics at your Battalion Aid Station.

If you wear glasses, you MUST have 2 pairs (1 MIL/1 CIV), Military Combat Eye Protection inserts with eye pro and gas mask inserts. Vision screenings will be conducted with all glasses and eye pro. Failure to produce your IME will result in a MEDPROS deficiency.

Dental:

Dental exams are available by appointment at the Okubo Dental Clinic. Call (253) 966-7670/967-4989 to schedule. Your appointment may be triaged based on the date of your last exam.

Physicals:

Ranger, SF, Airborne, HALO, SERE, Sapper, Air Assault or other Schools:
You must make an appointment. Check with unit medics for requirements needed prior to this appointment.

Flight: Flight Physicals are performed at Winder Clinic. Call (253) 477-0800.

ETS and Chapter:
For all Soldiers making a Veterans Affairs claim, make an appointment with the VA within 180 days of your ETS date.
For Soldiers not making a claim, make an appointment with Okubo within 180 days of separation date.

Retirement Physicals:
Retirement Physicals are completed by the Physical Exams clinic at the Madigan Annex. Appointments can be scheduled by calling the TRICARE Appointment Center, 1-800-404-4506.

PCS and Arms Room Screenings:
Are done at your unit Battalion Aid Station during Battalion designated PHA days and times.

Nutrition:

Self-refer appointments are conducted at Bldg. 13815. Please schedule at the Okubo clinic in person or by calling 253-966-1991.
