Directions

From the Madigan Army Medical Center Gate (Interstate 5, Exit 122) continue on Jackson Ave. Jackson Ave become Pendleton Ave. Make a left on 41st Division Drive. Continue on 41st Division Drive onto Railroad Ave. The 1-2 SBCT Soldier-Centered Medical Home is located at the corner of Railroad Ave and S 20th St.

About

The 1-2 SBCT Soldier Centered Medical Home (SCMH) offers a broad range of primary and specialty care services to active duty soldiers of the 1-2 SBCT.

Providers at the 1-2 SCMH are available for non-urgent medical consultation using the Army Medicine Secure Messaging Service, RelayHealth. Soldiers are encouraged to enroll in RelayHealth to

  • Discuss your current health and determine if you need to make an appointment
  • Discuss an existing or expiring profile
  • Request lab and radiology results
  • Request prescription refills

Soldiers can enroll in RelayHealth by logging on to www.RelayHealth.com

Sick Call

  • You must be screened by your battalion and receive a sick call slip.
  • If you need to be seen by a provider for sick call, report to the 1-2 SCMH between 6:30-7:30 a.m.
  • If you do not need to see a provider during sick call, but need to schedule an appointment, please call the TRICARE Appointment Line at 1-800-404-4506.

Walk-In Services

  • HIV draw
  • Immunizations
  • Pregnancy tests
  • Repeat medication injections
  • EKG

If you need to speak with your provider about an existing or expiring profile, please make an appointment or send a secure messaging through RelayHealth.

Behavioral Health

  • Behavioral Health services are available for Soldiers at the 1-2 SCMH
  • Behavioral Health services are also available for Soldiers at 1-2 SBCT Embedded Behavioral Health (EBH) located at R3742 Railroad Ave, JBLM, WA 98433.

Physicals

  • All chapter, school and ETS physicals will be scheduled at the 1-2 SCMH
  • Physical packets can be picked up at the 1-2 SCMH
  • Soldiers must complete all paperwork and labs prior to scheduling their appointment
  • Chapters 9, 10, 13 and 14 physicals require an escort
  • For flight and retirement physicals, please call the TRICARE Appointment Line to schedule

Physical Health Assessment (PHA)

  • All soldiers must complete Part I of their PHA online prior to making an appointment
  • All PHA appointments will be scheduled at the 1-2 SCMH
  • Soldiers over the age of 40 will need labs and an ECG prior to their appointment

Vision

  • If you need glasses, please schedule an appointment with the Winder Optometry Clinic at 253-477-3788. They are open Monday-Friday, 7-11 a.m. and 1-3 p.m.
  • You will be notified by your battalion when your glasses are ready for pick-up
  • Some soldiers are eligible to receive free refractive (laser) eye surgery at the Madigan Warfighter Refractive Surgery Clinic. You can make an appointment directly with the clinic by calling 253-968-5516.

Hearing

  • To complete your annual hearing exam, please call the Madigan Hearing Program at 253-968-3431.
  • You can also call the TRICARE Appointment Line and request a "well-hearing" exam.
