Physical Exams

Following are the types of physicals we perform, lab requirements and forms to be completed for each. You will receive a physical packet when you come in to get your labs done, there is no need to bring these forms with you.

Lab Requirements

A. Basic and Standard: No Fasting requirements.
B. Fasting: Must be fasting 12-14 hours prior to lab draw. Patient may only have WATER, no juice, COFFEE, SODA, etc. No food, gum, or mints. Patient can take medications, but only with water. If you have questions, please call 253-966-1991 Option 6.
