Clinical Practice Guidelines. This is the actual clinical guideline/practice parameter.  This might be an algorithm, text, check list, flow sheet, table, etc.

Key Points. This is a one-page handout that emphasizes the key points of the clinical standard/guideline.  The metrics (what will be audited) are also emphasized in this one-page key point summary.

Metrics. These metrics are the most important parts of the clinical standard/guideline, and all care providers must meet them when treating patients with the disease covered by the clinical standard.  The metrics will be audited to determine adherence to the clinical standard and whether providers are meeting the standard of care.

Provider Tools. Tools used to help providers when treating the disease covered by the clinical standard.  These include handouts, flow sheets, and check lists.  Not available for all of the standards.

Patient Education. Patient education material specific to the clinical standard/guideline, e.g. handouts, brochures, instruction sheets. 

POC/Authors. A list all members from the group (authors) that developed the clinical standard, and the point of contact (POC) for the clinical standard.  In some cases this person may differ from the first author of the document.

References. References used to ensure the Clinical Standard adheres to established best practice guidelines.

Core Document. The submission form/document that is completed and then used to present the clinical standard to the Clinical Standards Committee for review and approval.
